National Healthy Start HSMED

WFS is designed to help organizations case manage Healthy Start program and services. WFS supports the documentation and data collection that enables Healthy Start grantees meet the goals to reduce disparities in infant mortality and adverse perinatal outcomes.

WFS assists HRSA grantees in full case management of all participants from client referrals to enrolled participants.

WFS provides CW’s the ability to document all contacts, health history, risks, medical visits, educational services, additional screening/measurements, referrals to community partners.


Data Collection/Reporting

WFS uses a wholistic platform to support HS directors in achieving the many program goals that are required during the grant period. WFS supports HRSA grantees by collecting the HRSA form data, uploading the data to HSMED and providing the following reporting tools: 

  • Integrating HRSA data collection forms and other MCH assessments designed to guide programs toward improving women’s health

  • Collecting outcomes of the quality evidence based programs and services delivered

  • Documenting risk and protective factors designed to strengthen family resilience

  • Providing full Case Management System designed at achieving collective impact

  • Developing Reports that support increasing accountability through quality improvement, performance monitoring, and evaluation
